Executive Director / Administrator

Oversees all aspects of the community, ensuring integrity, compliance, and excellence in operations. With extensive experience in senior care management and regulatory oversight, the Administrator fosters a culture of respect, teamwork, and quality care.

Wellness Coordinator

Leads all clinical and care coordination efforts. Responsible for medication management, individualized care planning, and ensuring residents receive the highest level of health services according to Minnesota Department of Health standards.

Resident Care Coordinator

Acts as the main liaison between residents, families, and caregivers. Designs and monitors personalized care plans, ensuring comfort, safety, and satisfaction for every resident.

Activities and Life Enrichment Director

Creates and manages engaging activities that promote mental, physical, and emotional well-being. Organizes recreational programs, special events, and celebrations that bring joy and purpose to residents’ lives.

Culinary and Nutrition Manager

Leads the culinary team in preparing nutritious, home-cooked meals daily. Ensures each dish meets residents’ dietary needs and preferences while fostering a warm dining experience centered on comfort and community.

Support Staff and Caregivers

The heart of Gracewood Assisted Living LLC — providing 24-hour compassionate assistance with daily living, mobility, medication reminders, and companionship. Each caregiver is trained in emergency response, safety, and person-centered care.
